I had the same problem when I bought some GC camber plates off the old forum years ago.
I found out the seller didn't include the top hat or adapter nut and ended up sending everything in to Ground Control to be refurbished. Even then I didn't get instructions till I ask.
What they ended up sending was a crude drawing showing the camber plate/slider and adapter nut above the strut tower and the stainless ring/ bolts and top hat below the strut tower. No written instructions or torque specs.
Basically what you do is:
Push the bolts(attached to the stainless ring) through the holes in the strut tower, from below. Then place the camber plate, on top of the strut tower, over the protruding bolts and tighten it down with the nuts.
Slide the top hat over the strut shaft and seat it on the spring. Then slide the strut assembly up into the shock tower with the top hat seated against the slider and tighten in place with the adapter nut.
I know this is confusing with out pictures but I don't have any. I'm hoping someone else will post.
Best bet may be to call Ground Control. They seemed pretty good to work with.
